32 hrs/wk; 4 days/wk plus call requirement. Responsible for monitoring and reporting quality metrics and patient satisfaction for HH. Completing chart audits. Coordinate CMS and The Joint Commission updates and requirements and assist with survey readiness. Assist with referral management, triage phone calls. Provide utilization review. Act as liaison between agency and hospital/facilities. Will provide direct patient care 2 days/wk and when on call and SLC duties 2 days/wk. Clinical duties include wounds and wound vacs, IV antibiotics and TPN, phlebotomy, head to toe assessment, education to patient/family in patient's home or residence. Provide clinical support for field staff, patient scheduling, other managerial duties and audits. Must be able to work independently. Excellent communication and customer service skills, time management, strong computer skills, team player with previous nursing experience needed. Current NE RN, BSN license or willing to obtain, BLS. Able to drive own vehicle, valid driver's license and auto insurance. Prefer candidates with home health, hospice or case management experience.
